If you go to A&E for a standard toothache, it is likely you will have a long wait only to be given painkillers and advised to see your dentist. An urgent private dental clinic is usually more fruitful as they actually have the tools to fix most dental problems.
A&E departments (apart from teaching hospitals like Kings College Hospital, Denmark Hill and St Georges Hospital, Tooting) are not equipped to handle routine dental procedures (like fillings, extractions or re-cementing crowns).
However with these conditions you are advised to go A&E immediately:
- Severe swelling of your face, neck or jaw that is affecting your ability to close your eye or spread down your neck affecting breathing or swallowing.
- Uncontrollable bleeding in your mouth (after an extraction or caused by injury)
- Severe trauma to your face or jaw (like a suspected fracture)
- High fever or confusion (which are potential signs that a dental infection has spread into your blood stream)
While only an actual dentist can relieve the pressure causing this pain (using root canal or tooth extraction), there are some methods to help lessen the pain temporarily.
- OTC painkillers like ibuprofen (advised not to be used with pregnancy, uncontrollable asthma, heart disease and those with GI issues) and paracetamol (caution with liver disease); please consult a pharmacist before taking
- Immediate numbing agents like Clove Oil (Eugenol) or Benzocaine Gel (Oragel Extra Strength)
- Physical Relief like elevation of your head with pillows at night, Cold Compress with ice pack wrapped in towel on/off every 15 minutes, and warm saltwater rinses
You are advised to avoid extreme hot/cold food and drinks, and sugary foods as they are more likely to trigger the pain.
There is no exact length of time on how long a tooth nerve takes to die, but the progression usually follows a pattern.
It depends:
- If severe trauma occurred: (like trauma to the tooth with a hard hit) the nerve can die within a few days to 2 weeks.
- If the cause is dental decay or a slowly progressing infection: the nerve usually gives intense, throbbing pain for days or weeks before the nerve finally gives up.
Often after a few weeks or months, bacteria can multiply where the nerve has died, creating pressure at the end of the root and leading to a dental abscess. This is where your pain is more deep seated and like a heart beat throb, where the tooth often feels “high” or painful to bite on. This dental infection can spread into the jaw bone and cause a swollen cheek or jaw and needs treatment urgently.
Importantly a dead nerve will not heal itself. If the dead tissue is not removed via an extraction or root canal, the infection can spread to the bloodstream and cause sepsis.
A rotten tooth can stop hurting for a while, but typically the pain usually comes back. If the tooth has been hurting for days and then suddenly stops it usually means the nerve has died.
Do not assume your tooth is healed if it stops hurting after a period of intense pain. We advise that you get this checked with your dentist before a painful abscess develops.
